Major email providers

The services most of your users sign up with. Their own spam filtering catches most mail forged in their name, even where the domain's DNS alone would let it through.

Why these get their own list

Gmail, Outlook and the rest are free providers too, but they behave differently. Each runs several domains (googlemail.com, live.com, msn.com, ymail.com) and filters incoming mail at a scale a small provider cannot match. We keep a short, hand-picked list of them instead of guessing from DNS.

Questions

Does a Gmail address mean the signup is real?

It means the inbox is real and belongs to someone who went through Google's signup. It does not tell you who they are. Anyone can open several accounts, and Gmail ignores dots and anything after a plus sign, so one inbox can arrive under many spellings of the same address.

Are googlemail.com and gmail.com the same?

Yes. Both deliver to the same Gmail inboxes, so treat an address on either as one mailbox when you check for duplicate accounts.
